The Florida Department of Children and Families (DCF) and Department of Health require the following for every child enrolled in licensed child care or private school. We complete each item with you in person — no need to print or download.
Required by the Florida Department of Health for all children attending child care or school. Must be completed by your pediatrician and submitted before the first day of attendance.
A current physical exam form (within 12 months) signed by a Florida-licensed physician, ARNP, or PA. Required prior to enrollment.
Family contact information, child's full legal name, date of birth, allergies, dietary needs, and authorized pickup list.
At least two emergency contacts with current phone numbers and a list of adults authorized to pick up your child (with photo ID required at pickup).
Required for age verification and to confirm the child's legal name as it will appear on school records.
A valid Florida-issued driver's license or state ID for each enrolling parent or legal guardian.
These documents are reviewed and signed at the time of enrollment. We'll walk you through each one — no advance preparation required.
A signed acknowledgment that you have received and reviewed the Parent Handbook and facility policies, as required by Florida child care licensing rule 65C-22.
