We've gathered everything Florida parents need in one place — from state-required forms to health tips and important phone numbers. If anything is unclear, our office is just a call away.

Florida State Requirements

The Florida Department of Children and Families (DCF) and Department of Health require the following for every child enrolled in licensed child care or private school. We complete each item with you in person — no need to print or download.

DH 680 — Florida Certificate of Immunization

Required by the Florida Department of Health for all children attending child care or school. Must be completed by your pediatrician and submitted before the first day of attendance.

DH 3040 — Student Health Examination

A current physical exam form (within 12 months) signed by a Florida-licensed physician, ARNP, or PA. Required prior to enrollment.

Enrollment Application

Family contact information, child's full legal name, date of birth, allergies, dietary needs, and authorized pickup list.

Emergency Contact & Authorized Pickup

At least two emergency contacts with current phone numbers and a list of adults authorized to pick up your child (with photo ID required at pickup).

Birth Certificate (Copy)

Required for age verification and to confirm the child's legal name as it will appear on school records.

Proof of Florida ID — Parent / Guardian

A valid Florida-issued driver's license or state ID for each enrolling parent or legal guardian.

Enrollment Policies & Acknowledgments

These documents are reviewed and signed at the time of enrollment. We'll walk you through each one — no advance preparation required.

Acknowledgment of Receipt

A signed acknowledgment that you have received and reviewed the Parent Handbook and facility policies, as required by Florida child care licensing rule 65C-22.

Know Your Child Care Facility (DCF Brochure)

Florida Department of Children and Families brochure provided to every family at enrollment, explaining licensing standards and parent rights.

Rilya Wilson Act Notice

Florida law requiring child care providers to report when a child in protective supervision is absent. Families sign acknowledgment at enrollment.

Terms & Conditions of Enrollment

Tuition agreement, attendance expectations, withdrawal policy, late-pickup fees, and program-specific terms.

Welcome Letter

An introduction to your child's classroom, teacher, daily schedule, and what to bring during the first week.

VPK Parent Handbook (VPK families)

Voluntary Pre-K program guide covering Florida VPK attendance policy, curriculum standards, and assessments (FAST/STAR Early Literacy).

Health & Wellness Guidelines

  • Keep your child home with a fever of 100.4°F (38°C) or higher; they may return after 24 hours fever-free without medication.
  • Notify the office of any new allergies, medications, or medical diagnoses so we can update care plans.
  • All medications (prescription or OTC) require a signed Medication Authorization Form on file.
  • Children must be symptom-free of vomiting or diarrhea for 24 hours before returning to school.
