Step 1

Enrollment Form Requirements

Please submit the following documents with your enrollment application:

    Application

    Completed enrollment application form.

  2. 2

    Birth Certificate

    Official copy of the student's birth certificate.

  3. 3

    Most Recent Report Card

    Latest report card from the student's current school.

  4. 4

    Emergency Card

    Completed emergency contact information card.

  5. 5

    Exceptional Student Services Testing

    Testing records for gifted, learning disability, speech, etc., if applicable.

    Proof of Florida ID of the Parents

    Valid Florida-issued ID for the parent(s) or legal guardian(s).

Step 2

Required State Forms

The following documents are required prior to the first day of school:

    Current Immunization Form (HRS Form 680)

    Up-to-date immunization records on Florida HRS Form 680.

  • 7

    Proof of Physical

    Proof of physical from a Florida physician or the Florida Department of Health.
